After entering the intranet page, under the e-forms header (furthest to the right) click on the hyperlink labeled Stores Multi-Use Form (SMUF)
Print the form
After clicking on the link, the SMUF form will appear. It is a fillable form and you may fill in all blanks with the exception of the signature sections or just print it out and fill in necessary information.
Fill in order info
Only sections necessary to fill are:
PREPARED BY(your name),
APPROVED BY(supervisor signature),
CONTACT NAME (your name)
PHONE (office #)
DATE DESIRED(request 2 weeks after date of SMUF)
Scan SMUF form and attach the file
And, send to: Sergio.email@example.com.
